Referrals are welcome from physicians, therapists, and directly from clients.
Taking the first step in seeking help is often difficult. At our practice, we realize the importance of making the referral process simple and confidential. A secure online referral process is made easy to assist you in taking the first step.
Our process is efficient and confidential.
1. Click on the appropriate link and a new page will load.
2. Complete the form with relevant information. Please provide an email address appropriate for receiving an email from our office.
3. We will send a verification email (within the same day).
4. We will book an appointment with you via email. You will receive verification by email within 24-48 hours.
5. No phone call is needed. Your online referral is sufficient.
